Short Breaks Survey 2024

Overview

Welcome to the Short Breaks Survey 2024. This survey is aimed at Sefton parents/carers/guardians of children and young people with additional needs and disabilities that are entitled to, or access, Sefton short breaks services. 

There is also a Workbook for children and young people to complete.

Please note that at the end of this survey, before submitting your results, you will be asked if you want to provide an email address so that you can receive a PDF copy of your response.  This is completely optional.  The email address is only used to send the email to you via the online consultation system provided by Delib as the data processor.  The email address will not be stored with the responses and Sefton Council will not have access to it.  

Why your views matter

The purpose of this survey is to collate feedback on Sefton's short breaks services to inform future service development.

What happens next

The feedback received from the survey will be used to inform the future service development of Sefton's short breaks services for children, young people and their families.
