Dear Parents/Carers, 

We would like to gather your views, as parents/carers of children and young people with Special Educational Needs and/or Disabilities (SEND), on what is going well and what could be improved in our local area. 

We  would be grateful if you could complete this survey on Education, Health and Social Care services and the information and support you receive.

Your individual personal information will not be shared with services.

Please use a separate survey for each child or young person with SEND.

Why your views matter

We carried out the same survey in October 2019, as a baseline survey and the answers that you give in this survey will help us to identify how services are received and how you feel currently about the services and support, compared to 2019.

We want to then use the feedback to help identify what might be working well and where improvements need to be made.  

Your feedback is really important.  This survey should take between 15 - 20  minutes to complete.

By taking part in this survey, you will have the option later in the survey to be entered into a prize draw, where winners will be randomly selected to win one of three £50 shopping vouchers.

Please note that at the end of this survey, before submitting your results, you will be asked if you want to provide an email address so that you can receive a PDF copy of your response. This is completely optional. The email address is only used to send the email to you via the online consultation system provided by Delib as the data processor. The email address will not be stored with the responses, and Sefton Council will not have access it.  There may be a privacy notice within the Survey that will explain why we are asking for other information, and in this case, your email address, for the Prize draw.

What happens next

The responses will be shared with the SEND Board and relevant organisations/service providers to help understand what is working well and show what families feel need improving.
